How does the martingale part work?

The limited-slip loop closes a little when the dog pulls back, so the collar cannot slide over a narrow head the way a flat collar can. It only tightens to a set point.

Can I use it to restrain my dog in the car?

No. Attach a tether or car restraint to a harness, never a collar. This is a walking collar.

Is it cotton all the way through?

The printed cotton is the face, over a webbing core for strength, with metal hardware. The wide band is cut for narrow-headed breeds like whippets.
